Customer (name blocked for privacy), professionalism and confidentiality are the heart of the Employee Assistance Program's success. This program is 30 years old and I have never heard that someone would write such information on any document. You should report the use of this information to a top executive in your company. This is a violation of the program. You should notify in writing of the violations. You should request all information be removed from the document.
The only exceptions to confidentiality are those situations that specified under data privacy law. These laws require that counselors report to authorities when they have significant concerns over someone's safety, especially when that person is a child or vulnerable adult.
Everyone is given full explanation of the expectations and other data privacy practice at the time on intake. Also you know you can be anonymous in the EAP program. But you should raise this as a violation of your privacy. Your company should have a written policy that ensures confidentiality and you should request a full investigation why your confidentiality was violated You should address with the company that they failed to follow their own policy.
